NEW MEDICAL CARD REQUIREMENTS FOR CDL HOLDERS
EFFECTIVE January 1st, 2012.
All Commercial Driver License (CDL) applicants must certify with the Division of Motor Vehicles (DMV) whether they are:
- Engaged in interstate commerce and subject to Part 391 requirements; or
- Engaged in interstate commerce and excepted from Part 391 requirements; or
- Engaged in intrastate commerce and subject to State requirements; or
- Engaged in intrastate commerce and excepted from state requirements
All CDL applicants engaged in interstate or intrastate commerce and subject to Part 391 must provide an original or copy of their medical examiner’s certificate to DMV prior to issuance of a CDL (original, duplicate or renewal) beginning January 1, 2012.
Current CDL holders must comply with the certification and medical examiner’s certificate (if applicable) by January 30, 2014.
All CDL drivers must be in compliance by January 30, 2014
or their CDL priviledges will be cancelled.
You can self–certify beginning September 6, 2011 by filling out the self certification form at the link below and providing a copy of your medical examiner’s certificate (if applicable).

You may provide the required form(s) to DMV using any of the following methods:
- Hand deliver to your local DMV office.

DMV cannot accept faxed copies.
