NEW MEDICAL CARD REQUIREMENTS FOR CDL HOLDERS
EFFECTIVE January 1st, 2012.
All Commercial Driver License (CDL) applicants must certify with the Division of Motor Vehicles (DMV) whether they are:
All CDL applicants engaged in interstate or intrastate commerce and subject to Part 391 must provide an original or copy of their medical examiner’s certificate to DMV prior to issuance of a CDL (original, duplicate or renewal) beginning January 1, 2012.
Current CDL holders must comply with the certification and medical examiner’s certificate (if applicable) by January 30, 2014.
You can self–certify beginning September 6, 2011 by filling out the self certification form at the link below and providing a copy of your medical examiner’s certificate (if applicable).

DMV cannot accept faxed copies.
